Is it possible to have both celiac and Crohn's disease? I was diagnosed with celiac disease at the age of 3. I'm now 51 and my doctor says I have Crohn's as well. What are your suggestions to ease the symptoms of both diseases?
— Laurie, South Carolina
Both celiac disease, also known as celiac sprue or gluten-sensitive enteropathy, and Crohn's disease, one of the forms of inflammatory bowel disease, are relatively common diseases, particularly in most developed nations, so both can occur in one individual. However, it is important to make sure that your celiac sprue has not developed into ulcerative jejunitis, a complicated form of sprue that can mimic Crohn's disease on X-ray, as the treatment of celiac sprue and Crohn's disease are quite different. Most patients with celiac sprue become asymptomatic if they maintain a strict gluten-free diet. Since even a small amount of gluten can cause symptoms, you should consult a nutritionist or one of the many websites that give information about specific foods to avoid. Most health food stores and even some supermarkets now have gluten-free sections to help maintain this diet. A few patients with sprue do not respond to a gluten-free diet and are treated with steroids or other anti-inflammatory medications.
On the other hand, symptomatic Crohn's disease almost always requires anti-inflammatory medications or surgery for improvement. Unfortunately, no specific dietary recommendations seem to improve symptoms of Crohn's disease. Some patients keep a food diary to note certain foods that may make their symptoms worsen.
